EIGHT CAUSES OF HEEL PAIN

There are many form of foot pain experienced in my clinical practice, from these heel can be one of the most troubling one. usually pain in the heel is a common symptoms found in clinical practice especially for the patients who had age over 40yrs more on females.
pain in the heel can be classified into 3 according to the location of the pain.1.pain within the heel,2.pain behind the heel, 3-pain beneath the heel.
cause of these three types of pain are different.while a patient coming with a pain over heel,then should understand the exact location and cause, then only a physician can manage the disease.

1.Pain within the heel:

1.disease of calcaneous.
2.arthritis of subtalar joint

B.Pain behind the heel:

3.rupture of calcaneal tendon
4.calcaneal para tendinitis
5.post calcaneal bursitis
6.calcaneal apophysis.

C.Pain beneath the heel:

7.tendon heel pad.(may be due to calcaneal spur).
8.plantar fascitis.

some of the other causes are tarsal tunnel syndrome(TTS),arthritis,etc

Plantar fascitis:

Plantar Fascitis is a painful inflammatory process of the plantar fascia,connective tissue on the sole of the foot.it is often caused by over use of the plantar fascia or arch tendon of the foot.it is a very common condition and can be difficult to treat if not looked after properly.
patient always present this symptom as pain beneath the heel. diagnosed made by clinical examination.

Achillus tendinitis:

inflammation of the Achilles tendon at its insertion over the calcaneum causing posterior heel pain. patient always present the symptom as pain behind the heel.

Calcaneal para tendinitis or Calcaneal tenso vaginitis:

caused by inflammation of loose connective tissue para tendon, inflamed from excessive friction.present as pain in the region of calcaneal tendon, made worse be activities such as running or dancing.
on examination tenderness on palpation b/w fingers and thumb and deep to the tendon,and slight local thickening.

Post calcaneal bursitis or Retro calcaneal bursitis: 

inflammation of the bursa related to the calcaneum.present as pain behind the heel.an adventitious bursa forms at the back of the heel b/w tuberosity of calcaneus and skin.
o/e tenderness where the swelling is in contact.

Calcaneal Apophysitis or sever's disease:

only in children. between the age of 8yrs to 13yrs.pain behind the heel, slight limb may be noticed. tenderness over the lower posterior part of the tuberosity of the calcaneum.

Tendon heel pad:

pain beneath the hind part of the heel on standing or walking.mild inflammation of uncertain origin, well marked local tenderness on firm palpation on the heel pad.
